Once you have scheduled your appointment, we will need certain information from you as a new patient at South Florida Orthopaedics & Sports Medicine, including details about your current orthopaedic problem, your medical history, and insurance information.   For your convenience, our forms are available for you below. Simply download, print, and complete the appropriate form(s) prior to your arrival. This will help to speed up our new patient registration process. Otherwise, you are welcome to complete the forms when you arrive for your scheduled appointment.
